Christ Quest Ministries is a Christ-centered, non-profit ministry. A suggested donation of $85.00 is required at the time of scheduling an appointment which consists of 60 minutes, in person or by telecommunication.

I understand and recognize that Christ Quest Ministries Private Membership offers to disciple men/husbands that they will become the Christ-like spiritual leader God requires they be in order to care for their wife’s spirit in an understanding way according to Scripture, and helping a wife learn how to be the helpmeet God has designed her to be.

I understand and accept as a member that I am solely and completely responsible for the personal application of any and all information, instruction, recommendations or suggestions that I may receive.

I understand and recognize that the guidance received from Christ Quest Ministries Private Membership is NOT considered “psychiatry,” “psychology,” or “psychometrics.”

I understand and acknowledge it is my obligation to inform Christ Quest Ministries, prior to a session, that I do not desire to participate in spiritual care, but rather, I am seeking to establish a professional relationship with a licensed mental health service provider.

I accept and have fully informed myself of the contents of this Acknowledgment and Consent by reading and understanding it upon signing it.
